Port Jervis Census

2009 Census for All Children Residing in the Port Jervis City School District

In accordance with New York State Department of Education, the Port Jervis City School District is taking a census of all residents with children and we need your help. The purpose of this census is to assist us in planning for future student enrollment.

Please take a moment to list all children who reside at your address, including children under age five (5) and up to age eighteen (18). Please be sure to include all students attending any public, private, parochial, or home schools. All information will be held in strict confidence.

Please fill out the form below carefully. You can move back and forth between the tabbed sections of the form without losing any information previously entered. When you are done, please click the 'Submit Census' button once on the tab marked "Step 3" to send us your form. Please submit one form per family.
